A 52-year-old woman is admitted for a thyroidectomy, principal diagnosis nontoxic multinodular goitre. Recorded as that alone, the episode groups to K06B — Thyroid Interventions, Minor Complexity, at a cost weight of 1.84.
Document the comorbidities that were actually treated — asthma, type 2 diabetes, anaemia — and the identical episode groups to K06A, Major Complexity, at a cost weight of 3.57.

How does documentation quality change what we are paid?
- Directly, and usually by more than anything else under your control. A DRG is assigned from the coded record, so two admissions that were clinically identical but documented differently group to different classes and are paid differently. What is the difference between CDI and medical coding?
- Coding translates what the record says into classification codes. CDI is concerned with whether the record says everything it should in the first place. A coder working from an incomplete note will code it accurately and still under-represent the episode — which is why coding accuracy alone does not fix an under-documented record.
What is a physician query?
- A query is a request back to the treating clinician to clarify or complete something in the record — a diagnosis implied by the clinical indicators but never stated, or a condition treated but not documented. Queries are where CDI either works or annoys people, so they need to be specific, non-leading and few. CDI 360 manages them rather than leaving them to email.
Does it review every record, or a sample?
- Every record. Traditional CDI programmes sample, because a human team cannot read everything, which means the records that are never reviewed are invisible rather than known to be fine. Reviewing all of them is the part automation actually changes, and it is also what makes the compliance picture continuous rather than periodic.
